This urine test helps with the initial evaluation of patients with suspected Cushing syndrome (CS). Cushing’s syndrome (a.k.a hypercortisolism) is a condition that occurs from exposure to high cortisol levels for an extended period of time.
Evaluating a 24-hour urine specimen combines the production of cortisol for an entire day and is not affected by cortisol-binding globulin (CBG) levels even though serum cortisol levels fluctuate unpredictably and are strongly dependent on CBG levels. Urinary cortisol shows the portion of serum-free cortisol filtered by the kidney, and corresponds well with cortisol secretion rate.
Some symptoms of CS include high blood pressure, weight gain in the abdominial area, but with thin arms and legs, reddish stretch marks, a round red face, a fat lump between the shoulders, muscle weakness, bone weakness, acne and fragile skin. Women also may have more hair and irregular menstruation as a result of CS.

Patients must urinate at 8 a.m. and discard the specimen, then collect all urine in a 24-hour period, ending with the final collection at 8 a.m. the next morning.
