Follicle-stimulating hormone (FSH) is the pituitary signal that drives egg development in women and sperm production in men. FSH is one of the most clinically informative single hormone tests available โ it reveals ovarian reserve, confirms menopause, identifies premature ovarian insufficiency, and diagnoses male infertility causes in a single blood draw.

FSH Normal Ranges โ What Your Results Mean
| Population | Normal FSH Range | Clinical Meaning of Elevation |
|—|—|—|
| Women โ Day 3 Follicular | 3.5โ12.5 IU/L | Above 10: reduced ovarian reserve. Above 25: POI concern |
| Women โ Ovulation | 4.7โ21.5 IU/L | FSH briefly rises alongside LH surge |
| Women โ Luteal Phase | 1.7โ7.7 IU/L | Falls after ovulation |
| Women โ Postmenopausal | 25.8โ134.8 IU/L | Above 30: perimenopause. Above 40: confirms menopause |
| Men | 1.5โ12.4 IU/L | Above 12: testicular failure (primary hypogonadism) |
| Children (prepubertal) | Below 1 IU/L | Premature elevation triggers precocious puberty workup |
The Four Most Important Things FSH Tells You
FSH is produced by the anterior pituitary gland and travels through the bloodstream to stimulate the gonads โ the ovaries in women and the testes in men. Its level reflects the conversation between the brain and the reproductive organs.
Ovarian reserve in women โ FSH drawn on day 3 of the menstrual cycle is the standard measure of ovarian reserve. As the ovarian follicle pool depletes with age โ or from chemotherapy, autoimmune disease, or genetic factors โ the pituitary produces more FSH to try to stimulate the remaining follicles. Rising day 3 FSH above 10 IU/L indicates diminished ovarian reserve and reduced fertility. Above 25 IU/L suggests premature ovarian insufficiency. Day 3 FSH is one of the first tests ordered in any fertility evaluation.
Menopause confirmation โ FSH above 30 IU/L on two separate tests at least 30 days apart confirms menopause. FSH above 40 IU/L is considered diagnostic at most institutions. Estradiol below 30 pg/mL alongside high FSH confirms ovarian function has ceased.
Male infertility evaluation โ in men FSH stimulates Sertoli cells in the testes to support sperm production. High FSH
with low sperm count indicates primary testicular failure โ the testes are not producing sperm despite adequate pituitary
stimulation. This pattern suggests azoospermia or severe oligospermia requiring specialist evaluation. Low FSH with low sperm count indicates the problem is in the pituitary โ secondary hypogonadism that may be treatable with hormone therapy.
PCOS evaluation โ in PCOS FSH is typically normal or low while LH is elevated โ producing the characteristic LH/FSH ratio
above 2:1. FSH alone cannot diagnose PCOS but the LH/FSH ratio drawn together is one of the most specific blood markers
for the condition.

Frequently Asked Questions: FSH Blood Test
What is a normal FSH level?
Normal FSH varies significantly by sex and cycle phase. Women on day 3 of their cycle: 3.5 to 12.5 IU/L โ above 10 suggests reduced ovarian reserve. Postmenopausal women: 25.8 to 134.8 IU/L โ above 40 confirms menopause. Men: 1.5 to 12.4 IU/L โ above 12 suggests testicular failure. Cycle timing is critical for meaningful interpretation in women.
What does high FSH mean in women?
In women high FSH means the pituitary is working harder to stimulate the ovaries. Day 3 FSH above 10 IU/L indicates diminished ovarian reserve and reduced fertility. Above 25 IU/L suggests premature ovarian insufficiency. Above 30 to 40 IU/L on two tests 30 days apart confirms menopause. The higher the FSH the fewer responsive follicles remain.
What does FSH tell you about male fertility?
In men high FSH alongside low sperm count indicates primary testicular failure โ the testes are not producing sperm despite adequate pituitary stimulation. Low FSH with low sperm count indicates secondary hypogonadism โ the pituitary is not signaling adequately. These two patterns have different causes and require different treatments.
When should women get FSH tested?
For ovarian reserve, fertility evaluation, or PCOS assessment โ test on day 3 of your menstrual cycle. Day 1 is the first day of full menstrual flow. For menopause confirmation โ any time is acceptable, ideally two tests 30 days apart. Available through Personalabs without a doctor’s visit. Results in 24 to 48 hours. HSA and FSA eligible.
Can I order an FSH test without a doctor?
Yes. Personalabs provides the physician-approved lab order with your purchase. Order online, visit any Labcorp patient service center near you, results in your private secure account within 24 to 48 hours. No referral required. HSA and FSA eligible.
