Use:

This test is used to evaluate thyroid function and assess abnormal binding protein disorders.

This test particularly examines the amount of Triiodothyronine (T3) that is not attached in proteins in the bloodstream. The T3 in the bloodstream exists in two forms; one attached to proteins while the other is in unattached state (or free). Along with Thyroxine test (the T4) it is used to examine the overall health of your thyroid gland.

Why Free T3 Is the Most Important Thyroid Marker You’re Not Testing

TSH is the thyroid test most doctors order. It is also the least complete picture of how thyroid hormones are functioning at the cellular level.

TSH measures the pituitary’s signal to the thyroid โ€” not the actual hormone levels in circulation. A normal TSH tells you the pituitary is satisfied with what it’s seeing. It does not tell you how much active T3 is available to your tissues.

Free T3 is the only thyroid hormone that enters cells and produces metabolic effects. Your body cannot use T4 directly โ€” it must be converted to T3 by deiodinase enzymes in the liver, gut, and peripheral tissues. If this conversion is impaired, TSH and T4 remain normal while Free T3 falls โ€” a condition called T3 conversion disorder or euthyroid sick syndrome.

Conditions that impair T4 to T3 conversion: Chronic stress and elevated cortisol, selenium deficiency, zinc deficiency, insulin resistance, chronic inflammation, gut dysbiosis, caloric restriction and crash dieting, and aging itself.

People with T3 conversion disorder typically present with classic hypothyroid symptoms โ€” fatigue, weight gain, brain fog, hair thinning, cold intolerance, constipation โ€” despite being told their thyroid is “normal” because only TSH was tested.

What is Free T3 and why does it matter?

Free T3 (free triiodothyronine) is the active thyroid hormone that enters cells and drives metabolic functions including energy production, body temperature, heart rate, and cognitive function. Unlike T4 which is inactive until converted, Free T3 is the hormone that directly affects how you feel. Many people with normal TSH have low Free T3 due to impaired conversion โ€” this condition is only detectable with a full thyroid panel.


Can TSH be normal but Free T3 be low?

Yes โ€” this is called T3 conversion disorder or low T3 syndrome. Chronic stress, selenium deficiency, inflammation, insulin resistance, and restrictive dieting all impair the enzyme that converts T4 to T3. TSH remains normal because the pituitary monitors T4 levels โ€” not T3. The only way to identify this condition is to test Free T3 directly.


What are the symptoms of low Free T3?

Low Free T3 causes fatigue, weight gain despite normal eating, brain fog, hair thinning, cold hands and feet, constipation, slow heart rate, dry skin, and depression. These are identical to classic hypothyroidism symptoms but occur with normal TSH and T4 โ€” which is why they are frequently dismissed by doctors who only test TSH.
