Total Testosterone vs. Free Testosterone: Why Both Numbers Matter
Most men who get their testosterone checked only see a single number — Total Testosterone.
But that number alone can be misleading. Here’s what you actually need to know:
**Total Testosterone** measures ALL testosterone in your bloodstream, including the portion
bound to proteins (primarily SHBG — Sex Hormone Binding Globulin — and albumin). Bound
testosterone is inactive. Your body cannot use it. A man can have a ‘normal’ total
testosterone of 500 ng/dL and still feel every symptom of low T if his SHBG is elevated
and his free testosterone is suppressed.
**Free Testosterone** is the unbound, biologically active portion your body can actually
use. It typically represents only 1–3% of total testosterone. When free T is low — even
with a normal total T — men commonly experience fatigue, reduced libido, difficulty
building muscle, brain fog, and mood changes.

Understanding PSA: The Grey Zone and Why the Ratio Matters
PSA (Prostate-Specific Antigen) is the standard screening marker for prostate
health — but a single PSA number without context can cause unnecessary anxiety or
false reassurance. Here’s what most men aren’t told:
PSA levels 4.0–10.0 ng/mL are considered the “Grey Zone.” In this range, only
about 25% of men who undergo biopsy actually have prostate cancer — meaning 75%
of biopsies in this range come back negative. Yet many men in this range experience
unnecessary procedures and anxiety based on a single number.
The Free PSA Ratio changes the picture. Free PSA refers to PSA that is not bound
to proteins in the blood. When PSA is elevated due to benign prostatic hyperplasia
(BPH) or inflammation rather than cancer, the proportion of free PSA tends to be
higher.
A Free PSA Ratio below 10% is associated with a significantly higher risk of
prostate cancer and typically warrants biopsy. A ratio above 25% suggests benign
causes are more likely. This single ratio can dramatically reduce unnecessary
